Our protagonist had previously sought advice from an orthopedician, who earnestly advised him to quit smoking and prescribed necessary medications. Remarkably, the patient showed signs of improvement after he diligently followed the doctor’s advice, abstaining from tobacco and adhering to the prescribed medications. However, fate took an unfortunate turn when he relapsed into smoking and discontinued his medications.

A Devastating Diagnosis of gangrene

With the condition worsening, the patient sought help from a general surgeon. After thorough clinical investigations and evaluations, the diagnosis was made – gangrene of the right foot due to Peripheral Vascular Disease (PVD). This condition required immediate action, and as part of the treatment plan, the patient was advised to undergo metatarsal amputation of the affected foot under spinal anesthesia.

Here is the duplex study of both limbs

Duplex Study of Right Lower Limb Vessels

Venous system: Major deep and superficial veins of the right lower limb including common femoral, superficial femoral, and popliteal veins are normal in caliber, compressible, and show normal flow. No evidence of thrombosis is seen.

Arterial System: Common femoral, superficial femoral, popliteal, anterior tibial, and posterior tibial arteries are examined in the right lower limb. Diffuse thick plaques were noted at the anterior and posterior tibial artery causing near total to complete luminal occlusion. No color flow is seen within the lumen.

Comment: PAOD (Peripheral arterial occlusive disease) with –

Near total to complete luminal occlusion at right anterior and posterior tibial artery.

Duplex Study of Left Lower Limb Vessels

Venous system: Major deep and superficial veins of the left lower limb including common femoral, superficial femoral, and popliteal veins are normal in caliber, compressible, and show normal flow. No evidence of thrombosis is seen.

Arterial System: Common femoral, superficial femoral, popliteal, anterior tibial, and posterior tibial arteries are examined in the left lower limb. Mild wall thickening is seen in distal arteries. Mixed echogenic plaques were noted at the popliteal artery causing complete luminal occlusion. No color flow is seen within the lumen. Distal arteries show monophasic damped flow.

Comment: PAOD (Peripheral arterial occlusive disease) with –

  • Complete luminal occlusion at left popliteal artery.
  • Grossly damped distal flow.
